Launch COC & ensure TH5+
Tap Settings
Bottom-right corner gear icon
More Settings
Tap More Settings option
Change Name
Tap Change Name at top
Enter New Name
Type your name twice
Type CONFIRM
Type 'CONFIRM' when prompted
Tap Okay
Confirm to save changes
